Please note: Page 5 is a Criminal History Consent Form. Candidates must be willing to undergo a background check. When selected for the position, the candidate must have a physical examination and a drug screen. The county will pay for these tests and exams. The application must be notarized. staff members who are notaries at the County Administration building can notarize the application when you bring it to the building.
